PHASE 1: ELIGIBILITY INTAKE FORM
Companies aiming to apply for the technology assessment funding can start by submitting an intake form which would determine eligibility. CME will review the information and ask for supporting documents from the applicant. Eligibility will be determined based on the documentation provided by the applicant in conformance to the eligibility criteria.
PHASE 2: TECHNOLOGY ASSESSMENT APPLICATION FORM AND PROPOSAL
A CME representative will contact approved applicants and provide a copy of the Contribution Agreement (contract). Eligible companies will be connected with their Qualified Service Provider (QSP). The QSP will conduct an initial assessment and provide the applicant with details of the assessments process, findings and recommendations. The applicant will then need to submit a technology assessment application form
1. A signed copy of a proposal from a Qualified Service Provider (QSP) authorized by CME: The proposal must outline the assessment process, clearly articulating the areas of consideration and opportunities for improvement, the recommendations, and a description of how results will be measured.
2. A proposed budget: The budget should provide a detailed breakdown of costs, including time, travel, reporting, etc. All service provider invoices will need to clearly outline the services provided. Line item descriptions such as “services rendered” will not be accepted.
The submitted proposal will be reviewed by a panel after which the applicant will be notified if the proposal is approved or not. All approved proposals will receive an authorization to proceed letter. The applicant must then contact their QSP and set a date to start the assessment.
PHASE 3: ASSESSMENT
Applicants will have 90 days to complete the assessment and submit a report to CME for final eligibility for funding.
PHASE 4: FINAL ASSESSMENT REPORT & CLAIM
Once the assessment is complete, applicants will be required to complete an Expense Claim Form which will identify the budgeted expenses you submitted as part of your original application. Any significant variations (up or down) in actual expenses will need to be justified. CME representatives will provide the necessary forms to submit your reimbursement claim.
Once all expenses have been received, they will be reviewed for final eligibility. Approved expenses will be reimbursed within 30 business days of the approval date.
HOW TO APPLY
The technology assessment program requires the participating company to comply with the requirements of the four phases of the technology assessment program. Approvals with each phase will ensure that the company moves to the next phase of the program. The participating company will need to successfully complete all the requirements during the timeline of the program.
WHO SHOULD COMPLETE THE APPLICATION?
The contribution agreement should be signed by the organization’s signing officer. Third party submissions will not be considered.
HOW TO SUBMIT AN APPLICATION
This program ends on March 31, 2022. We continue to accept applications for assessments until December 31, 2021 with a completion date of no later than January 31, 2022. All reports and expense claims for approved assessments must be submitted no later than February 28, 2022. No extensions will be granted.
